The procedure for submitting this declaration is regulated by the Decree of the Government of the Russian Federation of December 31, 2005 No. 858. This resolution does not describe the procedure for filling out the document, but only approves the declaration forms. More detailed comments are given by the acts of the Ministry of Finance of the Russian Federation and the Federal Tax Service.
Instructions
Step 1
Write down the indicators of the volume of alcohol on a separate piece of paper in advance for yourself, and pay attention to the fact that they should be reflected exclusively in deciliters. You need to indicate these volumes in the declaration with an accuracy of three decimal places.
Step 2
Prepare a ballpoint or fountain pen with black or blue ink to fill out the declaration. It is unacceptable to use multi-colored ink for this purpose, but you have the right to fill out the declaration not in writing, but in printed form.
Step 3
Read carefully the names of the columns that are presented in the declaration. You need to enter only one indicator in each column and exactly the one that corresponds to the name of this column. In the case when the declaration contains such indicators that you do not have in reality, then you need to put a dash in this kind of column.
Step 4
Fill in the column "Declarant's name" in strict accordance with the constituent documents. To be more precise, here you need to write the organizational and legal form corresponding to the declarant, as well as the abbreviated name of the declarant himself.
Step 5
Indicate the code of the reason for tax registration of the declarant in the column "KPP of the declarant". Then you should indicate the place of state registration in the column entitled "Legal address of the declarant", and, accordingly, in the column "Actual address of the declarant" - the address of the direct location of the structural unit where the sale of alcohol is carried out under the license.
Step 6
Enter the series, registration number, as well as the start and end date of the license authorization for the retail sale of alcoholic beverages in the column "Declarant's license registration number, validity period". All this data can be found on the form of the license agreement itself.
Step 7
Fill in all the remaining fields in accordance with the characteristics of the alcoholic beverages that you sell in retail form in the structural unit indicated by you.
