Step 1

Start drawing up the act with the introductory part, in which you need to fill in several mandatory points. Name the document “Acceptance Act” by placing the heading in the center of the sheet at the top of the document. Leave the serial number and the date of writing the document below. Indicate the number of the contract according to which the work was carried out and the date of its conclusion. Mandatory for the indication are the details of the parties related to the customer and the contractor. Write for each of them the name of the organization with its registration data, legal and actual addresses.

Step 2

Fill in the mandatory items of the content of the certificate of completion. You can do this in the form of a table, in the columns of which it will be convenient to place information on the cost and amount of work. List all business transactions line by line, quantity, units of their measurement and cost. Fill in the totals, highlighting the VAT as a separate line. Duplicate the indicated amounts in words. If VAT in your case is not subject to allocation, in the column provided for this, indicate “without VAT”.

Step 3

Fill out the final part of the document by writing about the compliance of the work performed with the standards and requirements specified in the contract. In case of violation of these conditions, the identified deficiencies should be reported. If you have any complaints about the volume and timing of work, write about the non-compliance of the quality with the requirements specified in the contract. If the customer is satisfied with everything, write that the services were provided in full, and the parties have no complaints against each other. Leave a place for the signatures of the parties (authorized to sign the document) indicating their positions, as well as surname, name and patronymic. As agreed with the customer, issue a double-sided signature and affix seals from each organization.
