At the end, you need to fill in the column with the signatures of the parties and put the date of filling out the transfer acceptance certificate. Also, the act must indicate all the contacts of the performer: his phone number and the address of the organization (if you wish, you can add an email address).

Step 4

The acceptance certificate is filled in in duplicate, one remains with the customer, the second with the contractor. Keep it until the expiration of the warranty period. The seal of the organization must certainly be on the form of the act.
