Step 1

All entries in the certificate of incapacity for work are made in printed capital letters in writing or in electronic form. If the sheet is filled in in writing, then a gel or fountain pen is allowed, but not a ballpoint. The records should not go beyond the boundaries of the cells or touch them. In addition, the seal of the medical institution should also not be affixed to the space for the cells of the information field of the form.

Step 2

When filling out the back of the certificate of incapacity for work, indicate in the line "Place of work - name of organization" the abbreviated or full name of the organization, or the full name of the employer (individual). Put down in the appropriate lines the notes on the issuance of a certificate of incapacity for work for submission at the main place of work (or part-time). If you are issuing a sheet for submission for a part-time job, be sure to indicate the number of a similar document issued for the main place of work. If the disease occurred due to other reasons, mark it in the boxes of the column "Additional code".

Step 4

The subsection "Care" is filled out only if the certificate of incapacity for work is issued under the appropriate circumstances (the age of the sick family member and the degree of relationship are indicated). If parents take a sick leave for caring for 2 children, it is necessary to make notes about this. A second certificate of incapacity for work can be issued when caring for more than 2 children.

Step 5

In the line "Start to work" indicate the date of the citizen's exit to work (usually the next day after the last doctor's appointment). If the citizen continues to be ill, put down the code “31” in the “Other” column and issue a new sheet.

Step 6

Please note: the doctor must fill out only the spine of the certificate of incapacity for work and the section "To be completed by the doctor". Other information about the patient - SNILS number, TIN, as well as the number of the registration certificate of the organization - are indicated by the employer in a specially designated section.

Step 7

The certificate of incapacity for work is signed by a doctor, and later by the head of the organization and the chief accountant (or insured). The calculation of benefits for payments under the certificate of incapacity for work is carried out by the chief accountant (insured) on a separate form, which is attached to the main document.
