This includes diagnostic search, psychological assistance to the patient, and work with various papers. These are medical records, and strict reporting journals, and sick leaves, and all kinds of certificates. There is no doubt that properly executed documents are an indicator of good work. However, unfortunately, the documentation is not always kept flawlessly. Especially a lot of mistakes are made when filling out certificates.

Step 1
Take a blank form, get ready to fill it out, concentrate, do not combine this work at the same time with other things. Use only black or dark blue (purple) ink when writing help. Remember that most approved forms have two sides, a front and a back, both of which require proper clearance.

In the case of issuing student certificates in the appropriate column, indicate its serial number, which you can specify according to the register of issued documents. Sign the date of issue of the certificate, referring to the day the patient comes to you for help. Please note that when issuing a certificate retroactively, the date of its issue should not fall on weekends and holidays. Next, fully write down the last name, first name, patronymic and date of birth of the patient who needs a certificate. Then show the school, university or vocational school. The diagnosis of the disease in the certificates can be designated by a code according to the International Classification of Diseases (ICD) or in a short form, without specifying the phase, clinical form, complications. Remember the average morbidity and recovery time in each case. If there is no contact with infectious patients, simply put a dash in the section. At the bottom of the Help, set the date from which a student can and should attend an educational institution. At the same time, indicate for what period of time he is exempt from physical education. Be sure to certify this document with your signature, seals, in the upper right corner with a stamp, below - with the official seal of the institution that issued the certificate.

When issuing a certificate to the pool, please note that its validity period is six months. The surname, name and patronymic of the owner of the certificate should also be entered in full. Do not forget to add the applicant's year of birth. In the line "Diagnosis" enter "healthy (s)". At the bottom, mark the date, month and year of issue of the required document, sign.
