On January 1, 2006, a national project "Health" began to operate in all constituent entities of Russia, within the framework of which a program of birth certificates was developed to increase the material interest of medical workers and institutions in providing timely and high-quality medical care to pregnant women.

Instructions

Step 1

The certificate serves to pay for the work of medical personnel in the antenatal clinic, perinatal center and children's clinic, as well as to provide a pregnant woman with free medicines and to equip medical institutions with modern equipment. Payment for birth certificates is carried out from the budgetary funds of the Social Insurance Fund (FSS).

Step 2

When registering a sick leave for pregnancy and childbirth, a woman receives a birth certificate in an antenatal clinic. To obtain this document, you must see a doctor for at least 12 weeks. The certificate gives a pregnant woman the opportunity to choose not only an antenatal clinic, but also a maternity hospital.

Step 3

To obtain a certificate, you must submit the following documents to the antenatal clinic:

- passport or other document that certifies the identity of a pregnant woman;

- certificate of state pension insurance (SNILS):

- a policy of compulsory medical insurance (MHI).

Even if a pregnant woman does not have a medical policy or SNILS, a certificate must be issued, while the form indicates the reason for the lack of the necessary documents.

Step 4

A generic certificate is a document consisting of six parts: a spine, a certificate and four coupons. The first part is the back of the certificate, it confirms the issuance of the document and remains in the antenatal clinic that issued it to the pregnant woman. Coupon No. 1 is used to pay for medical services that were provided at a medical institution during pregnancy. This coupon is filled in at the antenatal clinic, and then transferred to the FSS for payment. Coupon No. 2 is used to pay for medical care, which was provided to a woman in the obstetric care facility. According to coupons No. 3-1 and 3-2, the services of a children's clinic are paid for, which monitors the child in the first year of his life. The birth certificate (without coupons) is given to the woman when she is discharged from the hospital, it indicates the weight and height of the baby.

Step 5

If a woman has been observed in a paid clinic for the entire period of pregnancy, a birth certificate is issued to her in consultation at the place of residence with a canceled coupon No. 1, stamped "not subject to payment" is affixed to it. In case of concluding an agreement with the perinatal center for the provision of paid services, coupon No. 2 is not paid.
