Step 1
As a rule, a pregnancy certificate is issued to a woman at her request. The stamp of a medical institution is placed in the upper right corner. It contains the full name of the medical organization, the name of the structural unit (department) in which the certificate is drawn up. The stamp contains the address of the location of the hospital (antenatal clinic), as well as the contact phone number.

In the middle of the document, capital letters indicate, for example: "examination room" or "certificate". Then the personal data of the woman who applied to the medical institution in order to obtain a pregnancy certificate is registered.

In the column where the patient's age is indicated, indicate the full date of her birth. Write the date, month and year in Arabic numerals in accordance with the information on the passport.

Enter the ultrasound results in the content of the certificate. Indicate the date of the ultrasound scan, write down the gestational age (the approximate period is indicated, since it is impossible to determine the exact one by ultrasound), write down the condition of the fetus. If it is normal, then put "N". Write the necessary recommendations if there are specific indications.
Step 5
Enter the real date of the pregnancy certificate. The document is signed by a gynecologist. His signature is certified by a personal seal, where his personal data and job title are registered.

Confirm the pregnancy certificate with the triangular seal of the medical institution, which contains all the details confirming the authenticity of the document.
Step 7
A pregnancy certificate can be requested by women in order to receive housing benefits, transfer to part-time work, to speed up marriage, and receive a deferral from the army. The certificate is submitted at the place of request and is a confirmation of the patient's pregnancy.
