Step 1

To pass the ITU, you need to issue a parcel sheet. This is a document that is filled out by the attending physician in the clinic. The parcel sheet contains the passport information of the patient, his full diagnosis, the results of the examination (conclusions of analyzes, instrumental research methods, conclusions of the necessary specialists). After the messenger sheet is completely filled out, the patient undergoes a medical commission (VC) at the clinic. VK identifies signs of disability and directs the person to the ITU.

Step 2

The patient's outpatient card (medical card from the polyclinic) is another medical document that is required when passing the ITU. Immediately before the meeting of the medical and social expert commission, the patient must pick up his card from the registry of the polyclinic and hand it over to the members of the commission.

If a person has undergone inpatient treatment or examination, he must submit to the commission the originals (or copies certified by the seal of the medical institution) of extracts from the hospital.

Step 4

Information about the nature of work activity - a document that the patient must bring from the place of work (if the person works). It includes a complete description of the type of work, the nature of the workload, and working conditions. The certificate must be certified by the seal of the organization.

Step 5

If a person does not work, he must bring the original work record book to the ITU. If the patient works, a certified copy of this document will be needed.

Step 6

A certificate of incapacity for work (sick leave) is required only if the person works. After the ITU meeting, an entry is made on the certificate of incapacity for work about the fact and the result of passing a medical examination.

Step 7

Passport - an identity document must be brought at a meeting of the medical and social expert commission.

Step 8

An individual rehabilitation program (IPR) must be brought only if the person undergoes re-examination, that is, re-issues a disability. IPR is a document that is issued by the ITU after the initial passage of the commission. It describes an individual rehabilitation plan for a disabled person (frequency of examination and treatment, names and number of necessary technical means, etc.). An individual rehabilitation program must contain special notes on its implementation (stamps, signatures of the relevant organizations).
