Requests for the provision of reference information imply its submission in the most convenient form for this. In case of reporting the results of any check, sometimes it is necessary to draw up a certificate. And here you need to remember that there is simply no unified form for it. But its arbitrary design is also unacceptable, since, in the absence of mandatory clauses, it can be invalidated.
Instructions
Step 1
The certificate is drawn up on the basis of the results of the check, recorded, as a rule, in the protocol or act. Therefore, take the original document and select the items in it that will need to be copied and transferred to the help. It is best not to change the text of these fragments, but strictly quote it in order to avoid accidental errors and discrepancies.
Step 2
Start the design of the introductory part by specifying the initial details. Copy the header of the protocol or act, which contains the name of the organization or enterprise. You do not need to do this if you have a corner stamp “for information”, which already lists the required details. Next, indicate the place and time of the inspection. After that, write the name of the document "Help" and immediately below it, briefly state the topic or essence of the information to be transferred.
Step 3
In the main part of the certificate, provide the information requested based on the results of the check. Most often, these are specific items that you can select from the main protocol, copy and paste into a new document. Such citation of the text will allow you not to disclose the full information obtained in the course of the commission's actions, but to highlight only the results of the information of interest.
Step 4
Now certify the certificate with the signature of the person authorized for such actions, decipher the signature (surname and initials) in brackets, indicate the position he holds at the enterprise. Affix a business seal if help will be provided to third parties. This is not required for internal documents.
