Step 1

Sick leave is issued at the clinic where the woman was registered for pregnancy. For this, an appendix to the order of the Ministry of Finance of the Russian Federation No. 172 dated March 17, 2007 is used. Enter the name, address of the medical institution. Write the medical organization code. Indicate on the form the personal data of the expectant mother. Enter the address of her registration in accordance with the passport. Write the name of the organization where it is performing the labor function. Indicate the date of issue of the sick leave.

Step 2

Enter the series, document number. In the field of the reason for the incapacity for work, write the maternity leave. Indicate the expected due date. In the mode column, write outpatient and inpatient.

Step 3

Count 70 days to and 70 days after birth. Indicate the period of maternity leave, which is 140 days for one fetal pregnancy, 194 for multiple pregnancies. Certify the certificate of incapacity for work with the signature of the attending physician, with the stamp of the medical organization. Give the document for signature to the head of the clinic.

Step 4

A certificate of incapacity for work is issued to a woman against receipt. The personnel officer of the company indicates the duration of the insurance experience. It includes the periods of work of the employee, when she actually performed her duties, and the employer made contributions to the pension fund of the Russian Federation.

Step 5

The accountant of the enterprise calculates the amount of the allowance. The amount of earnings for the 12 months preceding the date of the beginning of the sick leave is determined. The amount received is divided by the number of calendar days in the billing period. Average daily earnings are compared with the limit set by local government regulations. If it does not exceed a fixed amount, the daily wage is multiplied by 140.

Step 6

The amount received is paid on the back of the sick leave. At the expense of the employer, only the first two days of incapacity for work are paid, the rest is paid from the FSS of the Russian Federation. The amount of the allowance is included in the payroll when the salary is issued to the employee within 10 days from the date of the sick leave.
