If you are late for your workplace, where you must perform the labor function prescribed in the contract with the employer, for a certain amount of time, your immediate supervisor has the right to demand the writing of an explanatory note. In it, you need to indicate the reason for the delay and attach a document confirming its validity to the note.

Step 1
The upper left corner, as a rule, should contain the name of the company where you work, as well as personal information and the position of the head of the organization in the dative case. The upper right corner is the name of the department (service, structural unit) where you are registered.
Step 2
Under the name of the department, indicate the date of writing the explanatory note and its serial number (you can get it from the clerk). Write the subject of the note. In this case, it will correspond to being late for a certain amount of time.

Many enterprises have a developed explanatory memorandum form. In it you should put down the date and document number. The name of the organization and department (service) must be present initially.
Step 4
The substantive part of the explanatory note should begin, for example, with the words: "I, Elena Stepanovna Petrova, manager of the purchasing department …". Next, write the fact of being late for your workplace, indicate the amount of time you had to be late.
Step 5
Then you need to describe the reason why you did not show up on time for work. It must be respectful as well as truthful (after all, the employer can check it).
Step 6
It is advisable to attach a document confirming the validity of the reason for the delay to the explanatory note. Of course, if you have it in stock. For example, suppose you are late for work because of an accident on the way to your place of work. It can be confirmed by a certificate issued by the traffic police.

Write your position, personal data, sign. The explanatory note written by you should be sent for consideration to the director of the enterprise. He should make a decision: leave you in his workplace and consider the reason valid or impose disciplinary action.
