Step 1

Contact the tax office at the place of registration of the company and receive an application form in the UTII-1 form, as well as several annexes to it. Also, the form can be downloaded from the Internet by opening the official website of the Federal Tax Service of Russia or the regional office. Use these sources, as they publish the latest and most relevant information.

Step 2

Fill out the application in writing or electronically. Indicate the name of the organization, legal address, OGRN, TIN and KPP. Also fill in the boxes with the page number in the application, as well as in the annexes to it. In the first case, specify the value "001", then - "002" and so on. If you do not have copies of additional documents, put a dash in the appropriate box. When submitting an application directly by the head of the organization, put the number 3 in the appropriate box, and if by the representative, then the number 4.

Step 3

Indicate in the top line of the application the TIN of your organization. Also, in the appropriate field, indicate the separate TIN of the head or his official representative. Indicate in brackets that the application is made on the basis of a TIN assignment certificate. Fill out the sections by type of activity, indicate the reason for the organization's tax registration.

Step 4

If one of the employees submits the application, the manager must issue a power of attorney for him and certify it. Indicate the full name of the employee, his passport data, address and confidential actions (submission to the tax inspectorate of documents for registering the organization and signing the necessary papers). Next, the manager puts his signature and the seal of the organization at the bottom of the document. After that, all the collected papers are submitted to the appropriate authority to complete the tax registration procedure.
