Step 1
Create a file or a separate folder at home, where you will add contracts and payments made with all medical institutions throughout the year.

At the beginning of next year, sort the documents by medical institution and call them for a certificate for the tax authorities. Many medical institutions ask you to provide them with copies of payment documents and a number of other documents (copies of your passport or TIN). Sometimes you can negotiate with them and use e-mail. If a medical institution has a well-established electronic document flow, then they may not ask you for any data at all. Immediately discuss the terms of the certificate readiness and options for obtaining it. It so happens that a medical institution sends this document by mail, and sometimes they require a personal presence to receive a certificate.
Step 3
In addition to the certificate drawn up in the prescribed form, the medical institution encloses copies of the license of its organization. If you have not saved or have not received a contract for the provision of medical services, then you can try to request a duplicate of it.
Step 4
If you were in a spa treatment during the year and you were provided with medical services there, then these expenses also refer to expenses that are included in the social tax deduction for treatment. In this case, the form can be requested immediately on the last day of departure.
Step 5
Do not forget that paid treatment for your children is also included in the social tax deduction for treatment.
