To obtain a passport of a citizen of the Russian Federation upon reaching the age of fourteen, after changing the surname or replacing the lost one, prepare the following. First, this is an old passport. Secondly, a birth certificate with a mark of citizenship (for persons applying to the FMS for the first time). The third is a document confirming the change of surname (marriage certificate or certificate). In addition, you will need two photographs of 35x45 mm, color or black and white, and a receipt confirming the payment of the state duty for issuing a passport.

With a set of documents, go to the Office of the Federal Migration Service at the place of registration. If you are in another city, you can mark your passport at your place of residence. It's just that the registration process will take more time - one and a half to two months. This is due to the fact that first a request for confirmation of information will be sent to the FMS at the place of registration, and only after the answer will a new document be issued.

The FMS officer will ask you to fill out a questionnaire in which you need to indicate your old passport data, the reason for replacing the document, place and date of birth, last name, first name and patronymic (own and parents'), gender, registration address and region of receipt of the new document. On the back side, fill in the lines, adding there information about the presence or absence of a foreign passport. Its number and series can be found on the penultimate page of the civil document. Sign under this data and decipher the signature. If the passport is replaced in connection with marriage, write a separate application for changing the surname to another.

A new civil passport will be issued within a week if you change it at the FMS at the place of registration.

Remember that you need to change your civil passport within a month after its loss or expiration of its service life. Otherwise, you will face a considerable fine.
