write an application for registration of guardianship.

Prepare a document confirming your spouse's consent.

Make a copy of the person's birth certificate.

Prepare documents that confirm the absence of parents (copy of death certificate, etc.).
Step 5
Make a copy of your personal bank account, an extract from the house register at your place of residence and the place of residence of the child.

Prepare a certificate of your earnings.
Step 7
Issue your testimonial from your place of residence and work.
Step 8
Write your autobiography.
Step 9
Provide written consent of the ward (over ten years of age) to establish guardianship.

Issue a description of the child from the child care institution.

Prepare a health certificate for your child.

Submit an act of checking the living conditions of the child.

Provide an act of inventory of the ward's property.

Prepare certificates from medical institutions about your health condition:
- from the clinic, - from the neuropsychiatric, dermatovenerologic, narcological and anti-tuberculosis dispensaries.
Step 15
With the collected documents, contact the guardianship and guardianship committee, which is engaged in the establishment of custody of minors.
