Registration of a personal account card for the issuance of overalls

Personal accounting cards for the issuance of overalls, footwear, personal protective equipment (PPE) are drawn up in accordance with the approved form No. MB-6. This document fixes overalls, safety footwear and protective devices issued to employees at the established rate for personal use. The card is drawn up in one copy for each employee to whom the overalls are issued. In the document, fill in the lines "Full name", "Personnel number", "Height", "Sex", "Size of clothes, shoes, headgear", "Site", "Shop", "Profession", "Date of employment." Then you need to fill in the columns "Name of overalls, footwear and protective devices", "Paragraph of norms", "Unit of measurement", "Service life", "Quantity".

On the reverse side of the "Personal card" the plate "Issuance and return of overalls, special footwear and protective devices" is filled in. It indicates the name and nomenclature number of the issued overalls, in the "Issued" subsection, the date, quantity, percentage of validity, cost and signature of the employee upon receipt are indicated. In case of loss of overalls, shoes or protective devices in the section "Returned" on the line of the lost item, the act of disposal, its number and date are indicated. Duty overalls are issued with separate cards marked "Duty". The card must be signed by a safety engineer, shop manager, accountant. The document must be kept by the storekeeper of the shop (site). It is allowed to keep cards of workwear in electronic form using computer programs.

Who should draw up the registration cards for the issuance of overalls

The procedure for providing workers with overalls, as well as the procedure for storing, replacing, repairing, washing, drying, including the procedure for issuing personal accounting cards for the issuance of overalls, the employer must determine by a local regulatory act (regulation, order, organization standard). In this case, it is necessary to take into account the remoteness of the divisions from the central warehouse. The employer may assign the responsibility for the preparation of personal PPE registration cards to the head of the site, the head of the warehouse or other materially responsible employees. It is not allowed to impose the responsibility for keeping records of workwear on an occupational safety engineer, since, in accordance with his official duties, he must control the correctness of these documents (clause 7.23 of the Appendix to the Decree of the Ministry of Labor of Russia dated 2000-08-02 N14).
