Instructions

Step 1

The front side of the employee's disability sheet is filled in by a medical worker who enters the surname, name, patronymic of the specialist, the name of the organization at the main place of work (if a citizen has several of them, he works part-time). The doctor with whom this employee was on treatment writes in the start and end date of the sick leave, the number of calendar days of incapacity for work, the name of the disease, puts a personal signature, indicating the position held, his surname, initials. The certificate of incapacity for work must be certified by the seal of the medical institution.

Step 2

The back side of the certificate of incapacity for work submitted by the employee is filled in by a personnel officer. Indicate the name of the enterprise in accordance with the constituent documents or the last name, first name, patronymic of an individual, if the legal form of the company is an individual entrepreneur. Enter the name of the structural unit where the employee works, the position he occupies in accordance with the staffing table, as well as the personnel number in accordance with the employee's personal card.

Step 3

Indicate the dates of absence of the employee from the workplace and the date of his return to work. The head of the structural unit and the timekeeper (personnel worker) have the right to sign on the certificate of incapacity for work. Write the number of years, months, days of the specialist's insurance experience, depending on which the benefit paid to the employee at the enterprise is calculated.

Step 4

Enter the last name, first name, patronymic of the specialist, percentage of the salary for a certain number of calendar days of temporary disability. The disability benefit depends on the length of the insurance period. If it is less than 5 years, then the allowance will be equal to 50% of the salary, if from 5 to 8 years - 80%, over 8 years - 100%.

Step 5

Calculate the average daily earnings of an employee for a certain period. Multiply your actual earnings by the number of days actually worked.

Step 6

Calculate the average daily allowance of a specialist depending on the length of service, its maximum amount and depending on the minimum wage. The hospital employer must pay in an amount not less than the established minimum wage for the region.

Step 7

Indicate the amount of the benefit paid at the expense of your enterprise, the social insurance fund of the Russian Federation. After adding the above amounts, enter the total amount to be issued. Certify the certificate of incapacity for work with the signature of the chief accountant and the date of completion.
