After the most recent entry in the Job Details section, in the center, widest box, hand-write the full and, if available, abbreviated company name.

Below in the appropriate columns: the ordinal number of the entry, the date of employment, information about the employment with an indication of the position and, if necessary, divisions and output data (name, or abbreviated, number and date) of an order or other order for employment.

The employee's work book must be kept in the safe with the employer and returned to him on the day of dismissal. At his first request, the employer is obliged to provide a certified copy of this document.
