Who is filing the claim. For example, from Petrova Maria Ivanovna, living at the address: Moscow, Sverdlova Street, 20, apartment 220, phone: 00-00-00.
Step 3
Then write in the center of the sheet: CLAIM or STATEMENT.
Step 4
Next, on the line below, state the essence of the claim. For example, on February 22, 2011, I bought a washing machine in your store (indicate the brand and manufacturer).

Then describe the circumstances of the case and the nature of your claim, preferably referring to the articles of the law "On the protection of consumer rights".
Step 6
Next, state your requirements clearly and in detail. For example, I ask you to terminate the contract dated 02.22.2011 and return 15,000 rubles to me.
Step 7
At the end of your complaint, state your intentions if your claims are not met. For example, in case of non-satisfaction of the requirements presented by me on a voluntary basis, I will have to go to court with a request to satisfy the above requirements, to recover a penalty and compensation for moral damage.

Date and sign.

List the documents that you will attach to your claim.
