Step 1
Find out the address of the prosecutor's office in your area and the surname, name, patronymic of the prosecutor. These data can be clarified by first calling the prosecutor's office by phone or on the website of the relevant prosecutor's office on the Internet.
Step 2
Write your appeal to the prosecutor's office in the form of a statement or complaint. Determine for yourself the purpose of this appeal. This will help you build coherent text. It is better to print the document than to write it by hand.
Step 3
In the upper left corner of the application or complaint, indicate the name of the prosecutor's office you are applying to, for example, “The prosecutor's office of the Oktyabrskiy district of the city of Ensk”. Another option: indicate the surname and initials of the prosecutor, for example, “To the prosecutor of the Oktyabrsky district of the city of Ensk, VV Petrov”.
Step 4
Here, in the "header" of the document, indicate your last name, first name, patronymic, address where you live, contact information: phone number, fax number, e-mail.
Step 5
Below the "heading" in the middle of the sheet, write the word "Complaint" or "Statement", depending on the nature of the appeal. Instead of these words, you can address the prosecutor by name and patronymic, preceding them with the word "respected", for example, "Dear Vladimir Vasilyevich!"

In the text of the appeal, state the reasons why you are writing to the prosecutor's office, state the facts that you want to draw the attention of this law enforcement agency, indicate the evidence you have of an offense. At the end of your appeal, clearly state your request to the prosecutor. It is better to do this with reference to the norms of the law.
Step 7
If you want to attach any documents to the appeal, please list them in the appendix. It is better to send not originals, but copies of documents to the prosecutor's office. If necessary, you can submit the originals later, in a personal meeting with the prosecutor or his assistant.
Step 8
Sign the appeal, put the current date. If the document is drawn up on behalf of the organization, the signature of the head is certified by a seal.
