Step 1
After you have changed your name and issued a certificate of its change, you need to obtain a birth certificate for a new name. It is issued by the registry office to which you applied for a desire to change your name.
Step 2
Having received all the documents under the new name, contact the FMS. Write an application for a replacement passport. The application is filled out on a special form 1P. The application should be written in person in the presence of an FMS employee. He signs that the statement is correct and that the signature was put in his own hand.
Step 3
Attach passport photos, a passport with a former name, a certificate issued by the registry office about the change of name, a birth certificate for a new name to the application. If you are liable for military service in the reserve, then you must present a military ID. For persons with children - birth certificates of minor children. You will also need a certificate from the place of residence of registration and a document confirming marital status.
Step 4
A passport with a new name must be obtained within one month. If the citizen who changed his name does not receive a passport within the specified period, then he will be issued a large administrative fine in accordance with Article 19.15 of the RF Code.
