Instructions
Step 1
In order for you to have a changed name and surname entered in your passport, you must contact the registry office at the place where your personal data was changed and receive a new birth certificate or a certificate issued by the civil registration department that you have changed your surname or first name.
Step 2
You can also change the surname in the passport by presenting a marriage or divorce certificate, after which you changed your surname, taking your own, which you wore before marriage. The name cannot be changed by this method, for a change you will definitely need documents from the registry office.
Step 3
In addition to a certificate or a new birth certificate with changed data, present an application filled out on a unified form 1P in the presence of a migration service employee; old passport; military ID; a certificate from the place of residence on the composition of the family; birth certificates of children, if you have minors under 18 years of age; 4 photos 3, 5x4, 5.
Step 4
In ten days you will be given a new passport with changed data. In some regions, a passport is issued earlier or later than the specified period, it depends on the number of the population who applied for a passport change, as well as on the work of the regional migration services that check all the data.
Step 5
If children from 14 to 16 years old change their passports in connection with a change of surname or first name, then the parents, guardians or legal representatives of the minor must be present during the replacement.
