In addition to the application, the CMO must provide originals and copies of certain documents. So, to obtain an insurance policy for a child under 14 years old, you need to prepare a birth certificate, if you have SNILS and a passport or other document confirming the identity of the child's representative.

Citizens over 14 years old can independently obtain a compulsory medical insurance policy. They need to provide a document confirming their identity and in the presence of SNILS.

Refugees, foreign citizens and stateless persons living in the Russian Federation temporarily or permanently, persons without a fixed place of residence and occupation have the right to obtain an insurance policy. The list of documents that will be required for these categories of citizens can be clarified directly in the selected HIO.

It must be remembered that in the event of a change in the place or date of birth, gender or full name, the compulsory medical insurance policy must be replaced. In this case, the insured person is obliged to notify the HMO about the changes that have occurred within a month. If a person has lost the policy, then he must also notify the CMO about this and receive a duplicate of it.

It is possible to reissue and receive a duplicate of the insurance policy from the CMO on the basis of an application, while it is necessary to provide originals and copies of the above documents. On the day of filing this application, the CMO is obliged to provide a temporary certificate, which is valid for 30 calendar days and according to which a person can also receive the necessary medical care.
