The leave can be used by the employee for rest or other purposes not related to work. The grounds and conditions for granting leave may differ, but it is almost always mandatory for an employee to write a corresponding statement.

Instructions
Step 1
Fill in the header of your vacation application according to the “to whom - from whom” scheme. In the upper right corner, write with a capital letter, for example, "To the Director of LLC Vympel SS Sidorov", on the next line in capital letters: "from the engineer of the technical department Ivanov I. I." Then designate the name of the document - on a new line in the middle, write the word "application" with a small letter. Write the full name of the organization, the surname and initials of the manager, as well as your title and full name.
Step 2
Next, from the red line, state the main text of the application, where after the word "please" indicate: - type of leave (for example, main, additional, unpaid leave, parental leave, etc.); - the grounds for its provision (for example, the Labor Code of the Russian Federation, the organization's vacation schedule, collective agreement, etc.).
Step 3
If you are writing an application for unpaid leave, be sure to indicate the period of time for which you need to be released from work. For example, "I ask you to grant me unpaid leave from 01.10.2013 to 15.10.2013 due to family circumstances." By his resolution on your application, the manager will instruct the personnel department to take this period into account in the timesheet, and the accountant will take it into account when calculating wages.
Step 4
Sign and date at the very end of the application. If your organization is doing office work, send the application to the manager through the secretary, who will register it in the journal of incoming correspondence. In case of a positive decision on the application, the personnel worker will acquaint you with the order for granting leave.
