Step 1

Step 2

First, you must fill in the header of the form. Enter here the name of the organization in accordance with the constituent documents. If the vacation was postponed, then indicate the reason, for example, the employee's statement and the date of the expected vacation due (note that the employee must choose the time). In the tenth column, provide any notes.

Step 7

Sign the vacation schedule with the head of the HR department, then give it to the head of the organization for approval.
