There is no law or by-law establishing a uniform form of certification protocol for all enterprises and organizations. Therefore, the form is determined by the head of the organization. He creates the "Regulations on the certification of employees", which contains all the necessary information about the certification procedure, conditions, certification commission, as well as the final documents. Large firms develop and order special forms for such documents. The chairman of the certification commission and the secretary can be appointed by the management or elected, depending on the charter of the organization.

What is included in the protocol?
The protocol must contain all the data that are entered into such documents. This is the protocol number, the date of the certification, the list and data of those who pass the certification. The surname, name, patronymic, position of the participant, brief information about his report, questions that were asked to him, as well as answers are mandatory. If the protocol is a table, you can put information on the employee's participation in professional skill competitions in a separate column. The protocol should contain a special column on the voting results - how many members of the commission voted “for”, how many “against”. There are usually no abstainers at such events. In a separate column, the decision of the commission is entered on whether the employee's qualifications correspond to this category or category or not.
