A compulsory medical insurance policy guaranteeing assistance to a citizen of the Russian Federation can be issued through your employer, as well as at the place of residence (if at the time of registration of the document you do not have a job).
Necessary
- - the passport;
- - old-style policy;
- - statement;
- - SNILS.
Instructions
Step 1
To obtain a policy, you must contact the person responsible for this at work. Be sure to provide an old-style policy, as well as a passport. After submitting all the necessary documents, you will have to wait about a month before you receive a new compulsory medical insurance, which is valid throughout Russia. By law, you can choose a doctor and be served at the clinic you want.
Step 2
You also have the right to independently apply (in person or through a proxy) to any insurance company that has state accreditation to carry out such activities. In addition, you can submit an application to the territorial fund. Instead of a policy, you will be issued a temporary certificate for the time it is issued. Thanks to him, you will also be able to receive all types of medical care. As soon as the document is ready, the organization will personally issue a new policy to the applicant or his representative.
Step 3
You will need to attach certain types of documents required to register as an insured person with your application. Their list directly depends on the social status of a person (for example, a student, a pensioner), his age, whether he has Russian citizenship and many other factors. Minors under the age of 14 will only need to present a birth certificate. It is worth noting that only a representative (who has reached, naturally, 18 years of age) can apply for a minor.
Step 4
Adult citizens of the Russian Federation will need a passport or a temporary certificate issued for the period of issuing a new document. If there is SNILS, present it as well.
Step 5
Foreign citizens permanently residing in the territory of Russia must attach a residence permit to the application, as well as present an identity document recognized by an international treaty of the Russian Federation.
