  • - birth certificate of a child under 16 years old (if any),
  • - 2-3 photographs measuring 3, 5 x 4.5 cm.,
  • - 34 hryvnia.

Instructions

Step 1

Contact the ZhEK at your place of residence to obtain a certificate of registration (registration). The passport officer has a form for this certificate.

Step 2

If your passport has been stolen, write a statement to the police as soon as possible. This will reduce to zero the likelihood that someone will receive a loan using a stolen passport, register a fictitious enterprise, etc. Get from the police an extract from the criminal proceedings opened on the fact of theft of a passport and other property. You will need it to apply for a new passport. Also post information in the newspaper about the loss of your passport.

Step 3

Come to the reception at the department of the State Migration Service (former passport office) at your place of residence to obtain a new passport. Fill in the following documents there:

- statement about the loss of the passport with a description of the circumstances in which this happened;

- application for the issuance of a new passport of a citizen of Ukraine.

Step 4

In addition, to obtain a new passport, you will need:

- 2 or 3 (depending on the situation) photos;

- originals and copies of certificates of birth, marriage or its dissolution (if any);

- original and copies of birth certificates of children under 16 years old (if any);

- a certificate from the housing office about registration (registration);

- bank receipt for payment of state. fees for issuing a new passport (34 hryvnia).

If the passport was stolen, additionally provide a copy of the extract from the criminal proceedings certified by the police.

Step 5

Simultaneously with the submission of documents for obtaining a new passport, write an application for the issuance of a temporary identity card. After receiving the passport, hand over the certificate to the department of the state migration service. Having received a new passport, make sure that it contains all the necessary marks regarding registration at the place of residence, marital status, and the presence of minor children.
