A person who has reached the age of 14 can change his name, which includes the last name, first name and patronymic. Until a citizen reaches the specified age, nothing can be changed on his own. Instructions
Step 1
Contact the civil registry office at the place of residence or at the place of state registration of birth of the person who wants to change the last name, first name and patronymic.
Step 2
Write a statement about your desire to change your last name, first name or patronymic. It must indicate your full name, date and place of birth, citizenship, nationality, place of residence, marital status. If you have minor children - indicate the full name and date of birth of each existing child, the details of the civil status records compiled for you earlier, as well as in relation to each of the minor children. Sign the application and indicate the date it was issued.
Step 3
The change of name must be registered with the civil registry office. To register a change, write a statement. Your application will be considered within 1 month. In the absence of any documents, consideration may be delayed for up to 2 months. If the old civil status records are lost, then the registration of the change of the full name is made only after the restoration of the records in accordance with the law.
Step 4
If you are denied registration of a change in your name, you will be informed of the reason for the denial in writing. In case of a positive decision on registration, the civil registration authorities are obliged to report the registration to the federal executive authorities for control and supervision in the field of migration. The report must be made within seven days at your place of residence.
Step 5
To change the full name of a minor citizen under the age of 14, the consent of both parents, adoptive parents or guardians is required. In the absence of the consent of one of the child's legal representatives, the change is made only by a court decision.
