An extract from the work book is made by an employee of the organization's personnel department on the basis of a written application from the employee within three working days. This provision is enshrined in Art. 62 of the Labor Code of the Russian Federation, as well as clause 7 of the Rules for maintaining and storing work books, making forms of work books and providing employers with them, approved by Decree of the Government of the Russian Federation No. 225 "On work books" dated 04.16.2003.
The extract is drawn up on the letterhead of the organization, if any, and must be certified by the signature of the head, as well as the seal of the organization. The extract must contain the information indicated on the title page, as well as in the sections "Information about the work" and "Information about the awarding". An extract from the work book is provided to the employee free of charge.
When certifying the compliance of the extract with the original of the work book, below the requisite "Signature", a certification inscription is put down, consisting of the following elements: the inscription "True"; the position of the certifying person; personal signature of the certifying person; decryption of the signature (initials, surname); date of assurance. The organization may use a specially made seal with the text “Correct. Human Resources Inspector _ "or other similar text. But even if there is a seal, the signature of the certifier must be affixed by hand. The seal and signature of the authorized official are evidence of the correctness of the extract from the work book. The statement also contains the date of its issue and a note is made that the original document is in the given enterprise, institution.
