Instructions
Step 1
If you have lost your child's birth certificate or it has become unusable, contact the vital statistics office.
Step 2
Write a statement indicating the reason for the loss of your birth certificate. Show your passport, marriage or divorce certificate and a photocopy.
Step 3
In the application, indicate your passport data, full name of the child, place of registration and address of actual place of residence, date of birth, date of registration of the fact of birth in the registry office, place of registration.
Step 4
One of the parents, guardians or legal representatives of the child can apply for a duplicate, if they are not deprived of parental rights. Also, a duplicate can be obtained by an adult citizen himself, a notarized trustee of parents, guardians or legal representatives.
Step 5
If the first copy of the registry office record has been preserved, then the terms for issuing a duplicate will not exceed 30 days. When submitting an application, you will be given the day on which you can contact the registry office and receive a duplicate.
Step 6
Often there are situations when the first copy of the registry office is lost. In this case, apply to the local executive authorities, where there is a second copy of the civil status record. The deadline for obtaining a duplicate can take up to two months.
Step 7
If the registration of the fact of birth was carried out in another region, you can contact the place of your actual residence. The registry office will make a request, on the basis of which you can get a duplicate. The deadline for obtaining a duplicate of a birth certificate can take up to three months, especially if the first copy of the registry office is lost, and this happens not so rarely due to fire, floods, careless storage.
Step 8
A duplicate of a birth certificate is issued completely free of charge. Payment of the state fee is not provided.
